14-Jul-1988 - Only seven months after being rebuilt from its 1984 crash, the aircraft flew into the side of Battle Creek Mountain while operating a U.S. Forest Service flight from Redmond, OR to John Day, OR. The investigation determined the probable cause as acute in-flight incapacitation by the single pilot.
